Position Description

The Case Manger will provide comprehensive continuum services and individualized case management to address barriers to housing stability. The program staff will also provide in-home assistance and regular coaching through structured activities.

RESPONSIBILITIES
• Maintain a minimum of 42 qualified applicants into program annually
• Providing services to all program participants with housing needs
• Assist individuals with housing search via internet and in community (includes transportation of program participants)
• Network in the community monthly to obtain additional resources related to housing, employment, utility assistance, educational opportunities, etc.
• Assist qualified program participants with obtaining employment
• Assist qualified program participants with obtaining SSI/SSDI entitlement benefits through SOAR program model
• Assist participants by providing referrals or direct services
• Facilitate recovery and relapse prevention meetings as needed
• Conduct home visits weekly with participants
• Assist participants with ADL’s
• Complete ongoing assessments to determine client needs
• Complete monthly budget planning with residents
• Develop, update and track self-sufficiency goals monthly relating to housing and overall well-being
• Assist individuals in crisis by providing direct services or referrals
• Receive and document all additional information needed for hard copy file
• Participate in team staffing to discuss progress toward goals and self-sufficiency
• Receive and document all data needed for monthly and annual reporting in HMIS systems
• Maintain HMIS systems to include Case Worthy and Service Point APR documentation
• Provide transportation to clients as needed
• Conduct Housing Quality Standards Inspections annually as needed
• Complete required documentation for inspections (i.e. Inspection reports, notices to landlords, etc.)
• Assist qualified candidates with housing search
• Network monthly in community to obtain additional resources related to housing, employment, utility assistance, educational opportunities, etc.
• Coordinate services across St. Patrick Center (SPC) programs
• Update all appropriate Case Worthy HMIS categories for each client seen within 24 hours of service provided
• Maintain data in Case Worthy system for the Annual Progress Report
• Provide in-home Living Skills coaching
• Meet all performance measures outlined by grants
• Assist with coordination of agency-wide or seasonal projects (i.e. Thanksgiving and Christmas distribution)
• Assist and complete special case management projects as directed by Program Manager and Sr. Director

KNOWLEDGE AND EXPERIENCE
• Bachelor's degree in social work preferred
• Substance Use Certification preferred
• SOAR Certification preferred
• Knowledge of Housing First Model
• Experience working with low-income or homeless individuals/families (min. of 2 years)
• Experience working with chronic homeless, chemically dependent and individuals diagnosed with mental health conditions (min. 2 years)
• Basic knowledge of HUD programs
• Experience with wrap-around services preferred
• Experience/knowledge of housing placements
• Experience conducting home visits
• Understanding of area resources to provide quality referrals for the needs of clients
• Daily charting
