Position Description

The Integrated Treatment Services Case Manager at Rosati Place will oversee the daily care of residents in compliance with their identified treatment goals and assist in the implementation of treatment programs. The Case Manager will assist in direct implementation of all program goals for the Intensive Residential Program and related programming at Rosati Place.

• Develop and maintain a positive and therapeutic relationship with each resident
• Be familiar with each resident’s diagnosis, medications and history to assist in implementation of treatment goals
• Keep client files/paperwork organized and continue to update client files and internal files to reflect program and audit needs
• Conduct comprehensive and ongoing assessments of all resident’s mental illness, substance use, physical health, housing status and other relative needs in accordance with housing stability
• Understand different theories and modalities within client engagement including Housing First knowledge
• Provide direct clinical services to all residents on an individual and group basis in the office and community settings as needed and make appropriate changes to treatment plans to ensure the strengths, needs and resources of client
• Assist in creating a comfortable atmosphere for residents
• Assist in overall continued program development within Rosati and individual programming
• Treat each resident with respect and keep professional boundaries with residents and staff
• Report behavioral/physical changes, problems and/or concerns regarding residents to appropriate individuals and respond/intervene as needed in a therapeutic manner
• Maintain Group Home client case notes through BJC requirements and Case Worthy electronic system
• Update all client goals, outcomes and treatment plans into the Case Worthy system
• Keep an internal treatment plan for every resident, along with the BJC treatment plans
• Monitor the MAR for accuracy in coordination with Rosati Nurse
• Communicate regularly with supervisor for performance and feedback, as well as special assignments
• Perform any and all miscellaneous tasks as required or requested by Supervisor
• Other miscellaneous and special projects as assigned

• Master's degree in Social Work or Counseling/QMHP Required
• Understanding of mental illness and substance use
• Experience in counseling and case management
• High level of confidentiality, sound judgment and discretion
• Must be detail oriented, accurate and reliable
• Ability to multi task in a fast-paced team setting and ability to work independently
• Knowledge/understanding of area/community resources to provide appropriate referrals
• Knowledge of Federal, State and Agency policies of HIPAA confidentiality

• Excellent written and verbal communication skills
• Well versed regarding ethical issues
