Position Description

As part of the Assertive Community Treatment (ACT) multidisciplinary team, the Employment Specialist will provide treatment, rehabilitation, advocacy, supported employment, education and case management services to program clients with serious persistent mental illnesses and co-occurring substance use disorders.

RESPONSIBILITIES
• Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status and other relevant needs
• Manage primary case load by completing treatment plans, DLA-20 assessments and coordinating care through monthly client schedules
• Provide supported employment services to clients seeking competitive employment in the community (including coaching clients with job interview skills, assisting with job application paperwork and resume-creation and developing job opportunities including building relationships with employers in the community)
• Assist clients with making and keeping appointments and any other tasks that are required through the client recovery process
• Provide person centered direct clinical services to clients on an individual and group basis in the office and in community settings, making appropriate changes in treatment plans to ensure clients’ ability to cope with internal and external stressors
• Facilitate clients’ treatment planning with an emphasis on fostering personal growth and development towards recovery and wellness
• Refer clients to appropriate community agencies, assisting clients when needed
• Act as advocate and liaison, consulting with community agencies and program clients to maintain coordination in the treatment process
• Collaborate with other members of the ACT team during daily and weekly staff meetings
• Provide on-call crisis intervention in coordination with other ACT team members, covering nighttime hours, holidays and weekends
• Maintain St. Patrick Center and BJC-BH client tracking systems and client files (including treatment plans), documenting client progress to maintain a permanent record of client activity within the time frame required by DMH and BJC/administrative agent (24 hours)
• Be responsible for agency policy regarding work hours, building security and all written personnel standards
• Ensure the care model by interacting with all other St. Patrick Center programs
• Train other team members about supported employment services

KNOWLEDGE AND EXPERIENCE
• Master’s degree from an accredited university and/or five years of experience in related area; rehabilitation counseling training preferred
• Must be a Qualified Mental Health Professional
• Understanding of mental illness and substance use
• Experience in counseling and case management/wrap-around services
• Experience in conducting home visits
• Knowledge and understanding of community agencies
• Knowledge of federal, state and agency HIPAA policies
• Must possess current Class E Driver’s license (Missouri residents only) and reliable personal vehicle in which to transport clients

RELATIONSHIPS REQUIRED
• Report to and work with Manager of Behavioral Health Programs (Team Leader- ACT)
• Attend and participate in all appropriate St. Patrick Center meetings
