Intake and Retention Specialist - HERO

Position Description
SUMMARY OF POSITION:
This specialist will utilize wrap-around philosophy and techniques to assist clients who have successfully moved from transitional to permanent housing, self-sufficiency and stability in the community for a period of one (1) year. In addition, this specialist will coordinate all HERO intakes and admissions. In doing so, the specialist will meet with all potential admissions and gather all needed documentation. This specialist will work with all retention clients for up to one (1) year to provide follow up case management and assist clients with continued success in the community. This specialist will assist clients to maintain family and natural supports in the community, collaborate with the clients, Financial Stability Counselor (FSC), and the Substance Abuse Counselor (SAC) to track the success of their Individual Service Strategy/Treatment Plan of Care (ISS), coordinate individualized services for housing placement, coordinate the involvement of providers working in the home and community setting, and document all support related to activities and services.

RESPONSIBILITES:
• For a period of one year after transition to permanent housing, visit each client monthly to track ISS goals
• If unable to see the client fact to face, make phone calls to landlord, client, natural supports, employer, etc.
• Document each visit/attempt in Empowered Case Management (ECM)
• Provide support and resources as needed/available to aid clients in achieving ISS and treatment goals
• Contribute to the ideas and processes of the ICM team
• Assist with housing placement based on the desires of clients
• Work with FSC to address long-term financial stability needs
• Assist team members in addressing the needs of the clients by providing vouchers (e.g. transportation, clothing, furniture, food, etc.), hygiene kits, job referrals, or any other necessary referral (e.g. mentoring, substance abuse, mental health, advanced training opportunities, etc.)
• Coach clients to make solid, well-informed decisions
• Asses the clients’ strengths, desires, and immediate/long-term needs
• Develop or contact family and natural supports which exist in the community
• Collaborate with the wrap-around team that was developed with proper releases signed by the clients
• Receive input from ICM team members regarding employment, housing, mentoring, substance abuse, and other immediate/long-term needs
• Complete daily and monthly departmental paperwork
• Update daily clients’ electronic files in ECM and include any types of contact with client, services, or any internal or external referrals made
• Refer client to additional resources to address other service needs (i.e. legal assistance, VA benefits, community substance abuse counseling, etc.)
• Research additional benefits available to enhance service being provided
• Engage the client, the family and natural supports within the community setting (when feasible and desired by the clients)
• Meet with referrals weekly to determine eligibility
• If deemed ineligible, refer to appropriate program
• Complete all initial paperwork by providing informed consent of program guidelines and procedures
• Complete all property management paperwork
• Coordinate with VA on move in date
• Document intake and referral into (ECM)
• Must possess a Missouri Class E Driver License (MO residents only)
• Other duties as assigned

KNOWLEDGE AND EXPERIENCE:
• Bachelor’s degree from an accredited university or college; BSW preferred, or equivalent experience with proof of ability to perform effective case management
• Trauma informed care trained
• Knowledge of housing first
• Knowledge of harm reduction
• Knowledge of housing and other supportive service resources
• Available to work a minimum of 37.5 hours per week
• Reliable transportation is required
