Manager- Rosati CCT

Position Description
The Manager, as part of the CCT (Comprehensive Community Treatment) multidisciplinary team, provides treatment, rehabilitation, advocacy, and case management services to program clients with severe and persistent mental illnesses and chemical dependency problems. This position will identify individuals that meet the criteria of the goals and coordinate services for housing placements and coordinate all support services designed to keep them stable and housed.

• Lead the daily and weekly organizational staff meetings, continuously evaluating the status of program clients
• Implement and document quality assurance and research activities with the assistance of Manager of Central Intake/Quality Assessment and Performance Improvement
• Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status and other relevant needs
• Provide direct clinical services to clients on an individual and group basis in the office and in community settings, making appropriate changes in treatment plans to ensure the client’s ability to cope with internal and external stressors
• Reinforce long-term self-sufficiency goals and efforts and document progress
• Conduct and collect reports on 30-, 60-, and 90-day follow-ups on all program placements and document progress
• Refer clients to appropriate community agencies, assisting clients when needed
• Act as liaison and consult with community agencies and program clients to maintain coordination in the treatment process
• Collaborate with other members of the CCT team during daily and weekly staff meetings
• Participate in treatment planning groups, participating in setting treatment goals and plans with clients with an emphasis on facilitating personal growth and development towards recovery and wellness
• Provide on-call crisis intervention in coordination with other CCT team members, covering nighttime hours and weekends
• Maintain St. Patrick Center client tracking system and client files, documenting client progress to maintain a permanent record of client activity
• Complete and collect daily and monthly departmental paperwork
• Research additional benefits available to enhance services
• Coordinate and maintain client discharge records
• Implement and document quality assurance
• Complete expense reports, check request
• Responsible for the completion of departmental goals of case management as outlined by grants
• Assist with maintaining records and statistics for the completion of the Annual Report
• Assist with the coordination of agency-wide or seasonal projects such as School Supply Distribution, Thanksgiving Distribution, Christmas Distribution
• Ensure the care model by interacting with all other St. Patrick Center programs
• Cross train in group home responsibilities
• Must possess a Missouri Class E Driver License or be willing to obtain upon hire (MO residents only)
• Other duties as assigned

• Master’s degree in social work or counseling; LSW preferred
• Understanding of mental health conditions and chemical dependency
• Experience with wrap-around services preferred
• Experience in counseling and case management
• Experience in supervising staff
