Mental Health Counselor, Rosati CCT
Location: St. Louis, MO 63101

Position Description
As part of the Rosati CCT multidisciplinary team, the Mental Health Counselor is responsible for providing treatment, rehabilitation, advocacy, and case management services to program clients with severe and persistent mental illnesses and chemical dependency problems.

• Conduct comprehensive, ongoing assessments of program clients’ mental illness, physical health, housing status and other relevant needs, where appropriate
• Provide direct clinical services to clients on an individual and group basis in the office and in community settings, making appropriate changes in treatment plans to ensure the client’s ability to cope with internal and external stresses
• Refer clients to appropriate community agencies, assisting clients when needed (including transportation) and record in ECM
• Act as liaison and consult with community agencies and program clients to maintain coordination in the treatment process and record in ECM
• Collaborate with other members of the ACT team during daily and weekly staff meetings
• Facilitate treatment planning groups, participating in setting treatment goals and plans with clients with an emphasis on facilitating personal growth and development towards recovery and wellness
• Provide on-call crisis intervention in coordination with other ACT team members, covering nighttime hours and weekends
• Conduct 30-and 60 and 90 day outcomes on all program placement and document progress
• Complete daily and monthly departmental paperwork
• Maintain St. Patrick Center client tracking in ECM and HMIS systems, documenting client interactions and progress to maintain a permanent record of client activity
• Research additional benefits available to enhance services
• Responsible for the completion of departmental goals of case management to as outlined in grants
• Responsible for maintaining records and statistics regular and ongoing completion of annual reports and audits by program funders
• Assist with the coordination of agency-wide or seasonal projects such as School Supply Distribution, Thanksgiving Distribution, Christmas Distributions
• Be responsible for agency policy regarding work hours, building security and all written personnel standards
• Ensure the care model by interacting with all other St. Patrick Center programs
• Must have a Missouri Class E Drivers License (MO residents only)
• Other duties as assigned

• Master’s degree, MSW or related counseling degree
• Experience with mental health conditions and substance abuse
• Experience with wrap-around services, preferred
• Activity with conducting assessments and documenting home visits
• Knowledge and understanding of area/community resources to provide appropriate referrals
• Knowledge of federal, state and agency policies of HIPPA

• Report to and work with Manager, Rosati CCT and the Director of Rosati Center
• Attend and participate in all appropriate St. Patrick Center meetings

• Responsible for Mental Health counseling, case management, and transportation of clients
• Must have a reliable vehicle
• Ensure compliance with all City, State and Federal requirements regarding program
