Position Description

The Program Assistant, as part of the Hospital to Healthy Housing (H2HH) multidisciplinary team, will serve individuals experiencing homelessness and chronic health concerns. The Program Assistant will provide clerical support to Program Manager and team members as well as receptionist duties.

RESPONSIBILITIES
• Assist by managing housing stock and searching for open housing options
• Assist by communicating with landlords regarding paperwork and repairs
• Prepare check request and submit billing invoices as scheduled
• Prepare and maintain client file records
• Order necessary supplies for program/team members
• Assist by verifying benefits and scheduling appointments
• Operate and coordinate computer management information systems for program
• Sort mail
• Oversee client movement in building
• Assist in de-escalation and problem solving with clients
• Answer telephone and attend to client and staff issues in courteous person centered manner
• Communicate with team members in collaborative, interdisciplinary manner including attending all requested program/team meetings
• Maintain confidentiality (HIPAA)
• Miscellaneous support duties
• Utilize and coordinate housing efforts with case managers and staff at St. Patrick Center and in the community
• Provide education and support to landlords and clients as agreed upon by clients, which may include negotiating lease conditions and terms
• Develop and maintain professional relationships with landlords and property managers, advocating on behalf of participants
• Responsible for agency policy regarding work hours, building security and all written personnel standards
• Ensure the care model by interacting with all other St. Patrick Center programs
• Other duties as assigned

KNOWLEDGE AND EXPERIENCE
• Minimum Education Required: Associates of Applied Science in Behavioral Health Support, Community Health Worker, Human Services, Office Information Systems or General studies
• Preferred Bachelor’s degree in Human Services, Counseling, Social Work or related field
• Prior experience working with socially/economically disadvantaged persons
• Working knowledge of Microsoft Office and data entry experience
• Working knowledge of landlord-tenant laws and rights

• Proficiency in using Windows, Microsoft and Google Drive
• High organizational and time management skills
• Proficiency in written business communications
• Detail oriented
• Self-motivated, autonomous, reliable, collaborative within a team
• Sensitivity to cultural diversity of clients and staff
