Position Description

The case manager will utilize wrap-around philosophy and techniques to assist clients in transitional housing in the Grant Per Diem (GPD) program. The case manager will meet with clients to maintain family and natural supports in the community, collaborate and track the success of Individual Service Strategy/Treatment Plan of Care, coordinate individualized services with providers working in the home and community setting and document all support related activities and services with each client.

• Visit clients weekly to track Individual Service Strategy goals, updating as needed
• Work with other case managers (internal and external) to address long-term housing and financial stability needs
• Provide emotional and informational support for permanent housing by assessing the strengths, desires and immediate and long term needs of each client
• Conduct comprehensive and ongoing assessments of program participants mental illness, physical health, housing status and other relevant needs where appropriate
• Document, collect and maintain log sheets of client meeting attendance
• Conduct client drug and breath testing as needed
• Develop and/or contact family and natural supports in the community
• Receive input from team members regarding client employment, housing, mentoring, substance use, mental health and other immediate/long term needs
• Complete daily and monthly departmental paperwork
• Daily updates to client’s electronic files including types of contact with clients, services or any internal or external referrals made (within 24 hours of service provision)
• Adhere to strategies and goals outlined in the ISS and Treatment Plan
• Advocate for clients while providing coaching to make solid, well-informed decisions to maintain housing
• Assist client with monthly budgeting and provide financial/money management case management sessions
• Complete ISS and treatment plans, track goals weekly with client
• Must possess a Missouri Class E Driver License (MO residents only)
• Conduct and document 30 and 60 day follow-ups and beyond on program placements
• Provide client aftercare retention services for up to one year after exiting program
Perform Outreach
• Referring clients to additional resources to address other service needs (i.e. legal assistance,
VA benefits, community substance use counseling)
• Research additional benefits available to clients in effort to enhance services being provided
• Engage the client, family and natural supports within the community setting when applicable
• Any other duties assigned


• Bachelor’s degree required
• Peer Support Certification, MSW or related field preferred
• Ability to follow, understand and meet grant requirements, including managing paperwork and electronic documentation
• At least one year experience working in supportive housing
• Understanding of Harm Reduction and Housing First with knowledge of housing and other supportive service resources
• Ability to engage and work with community agencies and programs
• Reliable transportation is required
• Available to work a minimum of 37.5 hours a week

• Excellent computer skills and ability to process large amounts of data
