Position Description

The Case Manager will provide counseling and housing focused case management to the clients in the Women’s Night Program at St. Patrick Center. The Case Manager will ensure all activities with clients are consistent with their housing goals, engage clients and establish trust and collaborative relationships directed toward achieving client goals.
Work Sunday through Thursday 11:00am-7:00pm. Off Friday and Saturday.

• Work closely with the Manager of the Women’s Night Program (WNP)
• Conduct intake on shelter referrals
• Manage a caseload of 15 clients
• Provide individual counseling and housing focused case management for clients in WNP
• Crisis intervention with clients
• Conduct weekly house meetings for clients
• Maintain accurate filing system
• Input information into Case Worthy, HMIS and update regularly
• Efficient in HMIS and keep records updated
• Provide weekly updates on clients via matrix
• Collaborate with partner agencies to address the needs of clients
• Provide initial and ongoing assessment of clients’ progress towards housing opportunities
• Assists clients with creating short and long term housing and financial stability goals utilizing strengths and preferences as foundation on which to build goals
• Develops a housing stability plan with all clients
• Plan to include input from other treatment providers, family members and community supports as applicable
• Updates plan bi-weekly at minimum
• Assists clients with financial stability, including but not limited to obtaining employment and/or government benefits
• Accept shelter referrals on weekends
• Maintain integrity of the shelter on weekends
• Represent the manager in lieu of absence
• Rotate on-call weeks with the manager and other case manager
• Other duties as assigned

• Fosters strong working relationships with community based organizations and partner agencies
• Provides consistent communication with other organizations and partner agencies to ensure coordination of services
• Maintains database of current and community partners
• Participates in team meetings and communicates individually with team members to coordinate services and generate ideas to help clients achieve housing goals


• Bachelor’s Degree or Certified Peer Support Specialist required
• Experience working with mental illness and substance use
• Willingness to learn and gain more experience in above areas
• Trauma Informed Care trained

• Requires Class E Driver License (MO residents only)
• Requires reliable transportation
• Requires valid and current auto insurance
• Work a minimum of 37.5 hours a week

• Ability to handle difficult situations and people with calmness, respect and dignity
• Comfort and familiarity with St. Patrick Center client population
• Excellent written and verbal communication skills

• Attends quarterly night staff meetings with Manager
