Position Description

The Coordinated Entry (CE) Intake Specialist /Housing Navigator will provide front door and comprehensive intakes and assessments to individuals experiencing homelessness and divert from the homelessness whenever possible. The individual in this role will serve as an integral member of multi-disciplinary housing-focused service team. The Intake Specialist/Housing Navigator knows the housing market and is an expert in locating affordable units for clients building relationships and securing commitments from landlords that are willing to work with persons that have high vulnerabilities and barriers to housing.

RESPONSIBILITIES
• Complete CE Intakes for all individuals and families at-risk of or experiencing homelessness
• Ensure all paperwork is filled out properly by client and uploaded into Case Worthy
• Ensure clients have received appropriate resources and referrals at the time of intake
• Ensure clients are in both HMIS systems
• Complete quarterly training record
• Screen clients for prevention or diversion who are not literally homeless and refer them to services
• Accurately and appropriately perform VI-SPDAT assessments
• Ensure clients understand the CE process and next steps
• All clients are given a Participant Rights Packet
• Make 60 Day Follow-up calls to clients referred to the prioritization list
• Follow procedure for documenting contacts
• Maintain an “A” on the Data Quality Report Card
• Make appropriate and accurate referrals for clients into emergency shelter
• Will make appropriate referrals for clients to St. Patrick Center (SPC) and outside agencies
• Maintain active role in Student Training Program at Welcome Center through formal Field or Task Supervision or supportive position
• CoC/Agency Engagement by serving on at least one task force or committee
• Work with Reception and Client Services Monitor to ensure that client flow is maintained and runs smoothly
• Maintain up-to-date information on services provided by each program and Partner agency located at SPC
• Answer and/or direct incoming calls from potential clients and case workers from other agencies regarding services that SPC provides
• Participate fully and positively in a minimum of 1 training session bi-annually
• Participate in SPC Committees and/or SPC leadership opportunities within the agency
• Participate in fundraising activities for SPC
• Maintain Health and Safety Awareness reporting knowledge, MANDT Team participation and client and staff safety awareness
• Ability to de-escalate situations with clients
• Exhibit commitment to work ethic through punctuality and preparedness
• Accurately complete all personnel related documentation in a timely manner
• Communicate and interact professionally and positively with all clients, teammates, colleagues and others
• Establish landlord and property owner relationships for client benefit assessed through CE
• Create and maintain a database of landlords and rental properties
• Ability to conduct HUD Housing Inspections
• Visit available units
• Work closely with CE staff and housing providers to partner and share resources
• Collaborate with CE Case Mangers to assist clients with locating appropriate housing
• Attend Weekly Housing and Matching Meetings when needed
• Familiar with Missouri tenant rights and laws
• Familiar with application process for housing
• Other duties as assigned

DIRECT REPORTS
None

KNOWLEDGE AND EXPERIENCE
• Bachelor’s degree in Human Services, Counseling, Social Work or related field
• Experience working with socially/economically disadvantaged persons preferred
• Experience assisting with job placement and development activities preferred
• Working knowledge of Microsoft Office and data entry experience
• Must obtain an “E” Class Driver’s License or out of state equivalent
